You can set qmail to scan at queue-time with qmail-scanner or the spamc
wrapper that comes with SA.  Then qmail will scan every message that
passes through the server.  

Alternatively, you can scan on delivery by setting up qmail to deliver
to spamc, which in turn delivers to something like maildrop which can
take action based on SA's results.  Then you can turn filtering on/off
for individual mailboxes.  There is a tutorial for this kind of set up

The way I have SA set up, it only scans the message and adds a few
headers to the message.  The message's original subject and body are
preserved, so even if I were to scan at queue-time, the users who don't
want filtering would never know it's there.
