anybody have any idea how to catch something like this?

I've tried variations of header SUBJECT_ENCODED_MY_TEST Subject =~ /=\?.*\?=/
but it just ignores it.
By default QP encoding is decoded prior to running header rules, so you'll need to use Subject:raw instead of Subject.
