Can anyone tell me what I'm overlooking? Is there some magic switch required to get SpamAssassin to honor 'add_header' instructions?
First, run spamassassin --lint.. just as a sanity check.
Then I'd start checking very closely to be sure that the version of SA you *think* is running is the version that *IS* running.
Check for duplicate copies of spamc/spamd (ie: one in /usr/bin and one in /usr/local/bin).
Run spamassassin -v.. is it really 2.60?
Run spamassassin --lint -D and look at the debug output.. is it really parsing the rules directories you think it is? You've verified it's using your user_prefs.. but is it really using the same directory for the base rulset that you think it is?
