Now that I installed SA from scratch I was hoping to fix this. I have 'always_add_headers' and 'always_add_report' set to 1 and my
'spamassassin --lint -D' output looks normal with no errors.

Does SA require a perl module to re-write the headers or am I missing something simple here?

SA should honor the always_add_headers command.. and there's no perl module needed to do it, other than ones that are needed to run SA in the first place.

Are you sure there's not a "clear_headers" command in your local.cf or something?

does /usr/share/spamassassin/10_misc.cf contain the proper header declarations?

For 2.60, 10_misc.cf should contain:
add_header all Checker-Version SpamAssassin _VERSION_ (_SUBVERSION_) on _HOSTNAME_
add_header spam Flag _YESNOCAPS_
add_header all Level _STARS(*)_
add_header all Status "_YESNO_, hits=_HITS_ required=_REQD_ tests=_TESTS_ autolearn=_AUTOLEARN_ version=_VERSION_"
