The only "drawback" of MailScanner is it's double-queue mechanism. This is nice, because your same old MTA still handles all the mail transfer, but does have some disadvantages.

First, double queuing takes a slight amount of adjusting to in terms of a good feel for what it "should be like" so you can have a good sense for when things are going awry. If you set up from tarballs, it also takes a little bit of extra time to set up the queues in the first place as you have to switch out your sendmail startup script with a new one.

Double-queuing also does introduce some delay if you don't use "single" as your notify option (and "single" notify wastes some cpu when load is high). Of course, this delay has the advantage of slowing down a mail loop if you accidentally make one on your server (oooops).

Overall I personally like mailscanner. It's pretty flexible, and is wise enough to not try to re-invent a new MTA when it doesn't have to.
