When using mailscanner, all of the message modifications are done by 
mailscanner itself and SA is only used to generate a score. Ask on the 
mailscanner list and someone there should be able to help you if nobody 
here knows the answer.

I use mailscanner, but I'm running an older version so I'm not sure of the 
current syntax. I can tell you that it is able to do this, but spamassassin 
is not capable when run under mailscanner.
