I'm not really familiar with RH AS 2.1, but you should see if you have the
option of running sendmail 8.12.x on it.. apparently MILTER was pretty
quirky until the 8.12 releases.
you would probably get a lot of useful information if you consulted the
sendmail mailing list as well.
I personally use sendmail 8.12.9, MIMEDefang 2.35 and SpamAssassin 2.54
running on a RH 7.3 machine.
