After fighting this, I woke up this morning and bam, it hit me.  This is
how I was able to make a "spam" folder in a users exchange store without
doing it manually for every account:

- Open your inbox and create a new .pst file, call it whatever you want.
- Create a new folder inside that .pst called spam
- Put a message in that newly created spam folder, I sent an e-mail to
myself and moved it into the spam folder saying something like, "Welcome
to your new Spam folder, this is how you create a rule in outlook to
move spam to this folder...", complete with screenshots, etc.
- Go onto your exchange server as an admin that has rights to all
mailboxes.
- Install Exmerge.exe, this is a free util at MS
- Import from .pst file and choose the .pst file you created, make sure
you select merge or copy into mail store as the option.
- Wow, instantaneously, I have a spam folder in my inbox, complete with
a welcome message stating how I can use this spam folder.

Seems to easy but it worked well...
