I bet it's a problem with X-SA-Exim (which I have never used). It may be scanning messages twice. Once in X-SA-Exim where it is tagged as spam and marked-up, and once later on where you have your whitelist, doesn't flag it as spam, but keeps the original marked-up subject from X-SA-Exim. I can't be 100% certain, though, since I don't know your setup.
