You can invoke SA from Kmail, but I've found that doing so can make 
Kmail unresponsive, so I do it with procmail.

To invoke SA through Kmail, set up your first filter as "<any header> 
<matches regexp> .", then "pipe through" SA (with the full path to the 
SA script).

Once the message has been run through SA (by Kmail or procmail or 
other), select the blank header from the dropdown list of headers, 
enter "X-Spam-Status" into the blank, choose "matches regular expr.",  
enter "^Yes, hits=", and set the action to send it to your spam folder.
