I am running MIMEDefang+Spamassassin from /usr/ports on a 3 month old 4.6-STABLE. I merged/updated /etc/mail and /usr/share/sendmail/cf up to a current -STABLE. That might not have been necessary at all, but I wanted to make sure /etc/mail/Makefile was current. I did not need to recompile sendmail to get the milter working.
