I tried qmailscanner first, and while it worked, it's awful code (perl4 style) and it's really ugly to extend or improve. A better solution is qpsmtpd - a pure perl replacement for qmail's qmail-smtpd. I've been using it for about a month now, and I'm extremely happy with it, and I've even extended it with virus scanning (using clam-av).
