Resolved! I had originally installed SA from spamassassin-2.42-3.src.rpm so I decided to use the tarball Mail-SpamAssassin-2.42.tar.gz to reinstall.
After reinstalling SA I also reinstalled Qmail-scanner. SA is properly tagging messages now.
