I am using spamd with vpopmail and I have site wide settings on the 
local.cf file, and allow users to make changes to white/blacklists, subject 
rewrite, and a couple other settings via a webpage and this info is stored 
in a mysql database.

my spamd commandline is: /usr/local/bin/spamd -F0  -q -x -u qmails -v -a -m 
10 -s local1
I call spamc from maildrop using: xfilter "/usr/local/bin/spamc -f -u 
$EXT@$USER"

not all my users have spam filtering, so I do have a .qmail file for each 
(real/virtual) user that wants it.
