  I looked some more.  To duplicate the issue:

 - Use the standalone spamassassin 2.31 (not spam[cd])
 - Add these lines to a fresh user_prefs file:
score FREQ_SPAM_PHRASE  2 
score SPAM_PHRASES_020  3 
score SPAM_PHRASES_040  3 
score SPAM_PHRASES_100  3 

  Under these circumstances, SPAM_PHRASES_020 never scores.  If I delete the
user_prefs file, it works.  I haven't tested with the CVS version or
spam[cd].  I'll report this to bugzilla if anyone else can duplicate it.
