I've been doing a massive research project looking for a good web and mail hosting provider. I'm collecting my results here: <http://www.ii.com/internet/messaging/imap/isps/> The focus of the page is IMAP service providers but I think of IMAP as kind of litmus test for a web hosting service so there are some good web hosting services in the list too. I just signed up with dreamhost.com so I can't give you much of a report but so far they seem pretty good and they seem to have everything you're looking for. I especially like that they are running Debian Linux. Please let us know if you find any good ones that I can add to my list!
