One way it can be done is to call spamassassin/spamc from .forward or 
procmail, and then pipe the output to cyrus deliver with the proper 
arguments. Then you should be able to use sieve to filter based on Subject 
for example. Maybe even make a little wrapper around it all to make it 
easier for the users, so they don't have to enter several pages of text 
into their .forward ;-)
