Kevin, I think this is a useful feature to request.  Could you file a bugzilla
ticket if you haven't already, requesting a feature where the subject won't be
munged if it already contains the patterns that would be inserted?

As for the 2nd part, you might want to modify your procmailrc or however you're
invoking SA to *not* call SA if the subject contains "[RT ".  If you want to do
it within SA, then you can add a rule in /etc/mail/spamassassin/local.cf which
says:

header WILDBRAIN_RT        Subject =~ /[RT/
describe WILDBRAIN_RT      Claw back some points for ticket-tracking system
score WILDBRAIN_RT         -20.0
