I get application/octet-stream attachments all the time that are comletely innocent. Often they're patch files sent on the zsh mailing list or the like, but that's not the only case. There are any number of MUAs -- pine, for a unix example -- that offer the user no direct control over the type that is used for an attached file, and attach anything they don't think they recognize as a/o-s.
