Doesn't make much difference. The only thing they can do is turn
their spam into not-spam, but then they wouldn't be spammers any more

For user-specific customizations, yes. For site-wide use
/etc/spamassassin/local.cf (from the Debian package at least).

They are added to the collection of rules to check. If you specify a
rule with the same name as an existing one, then you "over write" it.
This way you can set custom scores on existing rules. You can also
create new rules by using a unique name.
