Various fixes to (re)enable PLUMPCMCIA_DEBUG build:
remove brackets around DPRINTF params (that's leftover from custom definition).
pcihp->ioh expects %lx specifier, not %x.
constify char *width_names.
rename cardtype to type in one of DPRINTF argument's condition.
