This really seems to work.
Now my Asterisk is only listening on the lan interface.
All inbound (wan) sip traffic is Natted to lan address.

DNAT    wan     lan:192.168.254.254:5060 udp    5060

Respondingtraffic goues out from the same interface (either ppp0 or ppp1)
