In both cases you just allow the tunnel program to act as a server. You
can also use "Only accept connections from the local machine" checkbox in
VNC4 (it is a registry entry in VNC3) to make sure only your tunnels can
get to VNC.

Using the Java applet doesn't help you get past any firewall that is in
place. Since you are using a proxy server, I would bet that you are being
blocked from connecting to the VNC server's RFB port.
Try doing a telnet and see if you get time outs there, too.
