This problem is very likely caused by some ISP rapidly switching IPs in the
same session thus what is recorded in the MAILDIR/*.qw file does not match
the subsequent requests.

yes.. (assuming you are using vpopmail) put a .qmailadmin-limits file into
the domain dir.. containg i.e.
maxpopaccounts 28
maxaliases 28
maxforwards 28
maxmailinglists 0
maxautoresponders 28

SQL table for each domain alias.
