It is quite likely that it was not in the database when you received it, but
was added

When I reran spamassassin -D on this message, it was flagged as spam

Qmail-Scanner doesn't call SA as "spamc < file" - look to see how it is
called and then run that by hand - you need to compare apples with apples
hint: spamc -f -u email@address
