and I created a relay_country.cf in /etc/mail/spamassassin
The setup I use is sendmail/spamassmilter/spamsassin(spamd) to reject mail
during reception.

OK, so it seems my inexperience with SMTP is the problem :-/
I can test only with telnet sessions, issuing SMTP commands by hand. I never
specified a 'To:' header -- I assumed it was added automatically from the
'mail from:' command.

Unfortunately, my only means of testing is through telnet connections from
network routers and not with a regular mail client. This is how I tested
things when I originally set up postfix and mail would be delivered with the
To: header intact.
