I created a document and inserted an index at page 2.
The index insertion dialog has 5 tabs. One is styles which is usually used to
define styles to be used for a specific level.
But in my case Level 4 [Contents 4] is shown which means "Contents 4" Style
format should be used. But it isn't. The
