I'm running apache on windows 2008 r2 and I'm seeing that some file handles
are being left open when using mod_cache_disk for some content. The I've
configured apache to cache all images locally as in my environment they reside
on a network share. This issue occurs after a number of days

user name that is passed by the proxy server.
I'm sure I'm missing something simple here, but I just can't seem to find
it.

fail2ban will do what you want.
It's typically used to add firewall rules based on logged events, but
could easily be configured to do whatever you want.
