Plack requires SCRIPT_NAME to be "" instead of "/" (
https://github.com/plack/Plack/issues/308).
Is there a way to do this with Apache 2.4.7?

Try adding L to your rewrite flags.
solved, webmin changed my NameVirtualHost, so the argument of
VirtualHost did not match
