Ik have not had any experience with Azure Key Vault myself, but I would be

Hi Luke,
I believe extensions are handled alphabetically. I had the same issue once
and then put an underscore before the extension name
(_guacamole-auth-openid).

Are you sure its Guacamole's fault? Did you try to connect with RDP directly
from another Windows machine?
