I'd like to modify my above comment because I discovered that the problem, for me, stems from KeepassX. It doesn't happen randomly, but only after the use of the global auto-type shortcut in KeepassX. If others are not using KeepassX, perhaps it is caused by whatever function KeepassX uses for it'
