I've also now tried your mountall in an lxc chroot, and it did not resolve my
issue there either.
I've set up a lxc container as described above, commented out the 'start
networking' and then:
$ sudo ln -sf console /path/to/rootfs/dev/kmsg
$ sudo lxc-start --name=$REL-amd64 --console conso

ubuntu downstream packaging to the ticket. If there's some change that

I just spent a few minutes trying to figure out the ordering based on the
scripts for Andrew's system, it's basically:
- eth0 appears
- triggers udev
- triggers upstart
- triggers ifup eth0
- triggers bonding
- bond0 appears
- triggers udev

Rationale for not touching vlan is that it's never calling ifup and so
shouldn't be able to cause a similar deadlock situation as bridge-utils.

Do you see any output during first boot which would indicate why tgt
does not present the LUN? I think this may be some sort of race
condition between tgt startup and the device mapper having detected all
devices.
It would be good if you could also try using a file as a backing-store
to
looked at dvipng. the package is bug free, the code looks ok, maintained
in debian. the package provides one binary, which is used as a build
