I've had this same issue with the 9.04 2.6.28-13 to 2.6.28-14 update.
I did the following to fix the issue:
Enabled the 'Alternate Atheros "madwifi" driver. Reboot. Not working
Disabled the 'Alternate Atheros "madwifi" driver. Reboot. Not working
Disabled wireless in the BIOS. Reboot. Not worki

Steps to reproduce:
1) Have a HD with an XFS partition that you want to keep.
2) Do manual partition and allow the pre-existing XFS partition to be mounted
Attaching requested files. Note that I could not reproduce this bug
when running "ubiquity --debug" but I did see the lockup when I added
--debug to the desktop launcher.

It seems to work now.
But I did receive a message dialog telling me that there has an issue
unmounting the directory /mnt/migrationassistant. (see attachment. The
attached image seems to missing some text, but don't remember any text
missing on the screen)
