I have tested in 8.04 (I only use the LTS version of Ubuntu). When you
run the X version of Bastille with 800X600 the buttons are there. At
640X480 the buttons are not. If you run Bastille w/o the X GUI (bastille
-c) the prompts are there in both 800X600 and 640X480.

Please find attached a copy of dmesg. I tried it twice and have attached
both as they appear slightly different whendetecting the usb hub but I
don't know enough to tell if this is relevant.
The second one also contains the repeated error message which is sent.

I have exactly the same problem. Printouts are identical.
Error messages stop when ehci_hcd is removed but am still unable to
mount usb. I have looked around to find a solution and it seems this is
a reasonably common problem.

Problem installing Xenial most recent daily desktop AMD64 builds (3
previous days) while selecting English keyboard and Ecuador timezone.
Same problem of crash during setting language.
Workaround was updating sources.list, update, reinstall apt tzdata
language-selector-common language-pack-en-bas

workaround: use PulseAudio Volume control, don't use ubuntu packaged volume
control. replug HDMI at TV.

Repro is quite simple, place a file in `/etc/skel/` who's filename ends with
the space character.
When running `adduser`, an error `No such file or directory` will be thrown.
Instructions:
$ sudo touch "/etc/skel/file-with-space "
$ sudo adduser test

received from Facebook and Twitter accounts (in my specific case).
