Same bug here on a 16.04, agw3's workaround doesn't fix the issue for me.
It occurs when my browser (firefox 53.0.2 64b) display a google research with
google translate content.
The way I fix it is:

Same problem here.
In fact, I no longer run Trusty on bare metal ; instead, I run up-to-
date 64bit Trusty userland in a chroot on up-to-date 64bit Yakkety.
The game is playable, except for this exact same power jump
sequence/combo.

The problem of this "solution" is that is temporary, because on each
boot you need to set manually the output on alsamixer (because
pulseaudio can't change it)
The driver developer Clemens Ladisch said me that it's because of a
difference between the name of the output purposed by the driver and t
's why I think this is races which cause THE BUG.

I want to compile my library (using SFML), all works on Ubuntu 14.04
LTS, but on Ubuntu 16.04 LTS (after recompile all libs) I have this bug.
Note that: If you use versions compiled on Ubuntu 14.04 and try to run it, it
works.

It seems o2cb resource is not needed any more. All the modules and setup
is done by the mount.ocfs2 command in the ocfs2-tools-1.8.4 working with
dlm-4.0.5 and a recent kernel. As a result you only need two resources
to mount the OCFS2 filesystem:
* ocf:pacemaker:controld
* ocf:heartbeat:Filesyste
I also am affected by the exact same bug with up-to-date ubuntu 16.04, as of
per today untity version is 7.4.0+16.04.20 .
I would also add that the launchbar doesn't track open applications windows
either, even when launched using said launchbar buttons.
