I can confirm the bug with the ASUS Formula V and Ubuntu 12.04, amd64 kernel 3.2.0-27. Upgrading the BIOS to 1503 solved the issue for me. Asus indicates that the BIOS "improves USB compatibility". Note that upgrading the BIOS will require you to re-install grub-efi because NVRAM is erased. -- Y
