I have probably the same problem trying to upgrade a server behind a proxy
using do-release-upgrade.
proxy is set using http_proxy environment variable.

It occur randomly only one or two time a week, other boot are fine.

Same problem for me with the 32bit game installer that use Gtk.
My ugly fix was to temporarily link /usr/lib to lib32 :-(

The "sleep 1" in kdm.conf also fix the problem for me.
