I have the same problem on Hardy LTS (upgraded from Dapper LTS).
It seems that physdev-out works with FORWARD, but dont works with OUTPUT.

Sure i have hardy installed. This is the last Ubuntu which works with my laptop
:-)
I did all tests you asked for. Hardy use intel driver by default. When
changed to i810 the resolution went down to 1024x768.

I have old NV18 [GeForce4 MX 440 AGP 8x] with 64 MB of RAM.
I have fixed my black window bug (appearing after 10 windows opened) by
switching to Xgl Xserver:
sudo apt-get install xserver-xgl
After logout and login I no longer have black window problem (50 windows
opened).
