Paul, one caveat with the firmware - it won't be reloaded onto cards
that are still in a "warm" state and USB cards often retain that status
even after a reboot - if you look at dmesg you can usually see a message
along the lines of "warm state, firmware not uploaded" on some such.

I have exact same problem - install kolab which put a bogus config into
apache2 causing it to fail. Can't remove ldap-account-manager because
that restarts apache2, which fails.
