is an issue in Ubuntu 4.0.1 build, but not in the upstream 4.0.2 build..

it is triggered on a more rarely used scenario, while it was more
important in 13.04 since the new shutdown dialogues highly highlighted
it.

I can' consistently make this happen, but I notice it most often when I
have a Nautilus file browsing window open and then insert a USB device

Ah, yes, I see what's happening now. The pam_usb module is granting
access without a prompt as soon as lightdm spawns which lightdm caches
even when you remove the token.
