I can switch off headers and footers during printing only to reboot system.
After reboot headers and footers appears again.
In previous versions of firefox it was global option and switched off once
remains switched off.
It's very annoying.

One other thing to try is to force the unloading/loading of the sdhci
and ricoh_mmc drivers over the suspend/resume. I suggest the following
workaround:
sudo gedit /etc/default/acpi-support
and change the MODULES="" line to MODULES="sdhci_pci sdhci richoh_mmc "
let us know if this helps.

I have a similar problem in karmic with KDE 4.4 installed (from kubuntu-
ppa backports).
However, shared-desktop-ontologies was already installed by default.
