Unity launcher single missing/invisible/blank icon

I did not spot the exact moment when it happened but I did not add
anything to launcher.
On the right click there was a context menu: the place for application
name was empty and there was 2 default options for running application -
"Unlock from Launcher" and "Quit". Did you know that screenshot c
seems similar to bug #983048
