I initially thought it was crashing but realized after filing the bug
report that it was having problems loading. By trying to run the
application from the command line per your instruction I discovered that
the gnome applets package was no longer installed. I tried to apt-get

The title is confusing if it's not crashing ;) Could you run /usr/lib
/gnome-applets/mixer_applet2 from a command line then try to add the
applet to the panel and note to a comment what is printed on the command
line?

I have not been able to get a backtrace because I don't think that
program is crashing but rather it won't load. The same error will occur
when you either log in or try to add the volume or mixer applets to the
panel.
