This seems to be working correctly now, in Ubuntu 16.04 (mpd still uses
ALSA by default, but uses the PulseAudio plugin, so it shows up under
Applications in the Sound control panel, listed as "mpd (ALSA plug-
in)").

I suspect the issue lies with PulseAudio being started from the user
session upon login, and a daemonized mpd missing some crucial bit of
information (pulse cookie from environment?) somewhere. Note that you're
not actually using ALSA, but the ALSA emulation provided by
pulseaudio...
