This happens to me a lot on Precise (with Unity-2D) when doing full maximizing.
I do have different monitor resolutions.
If it helps, I've been able to work around it a few times by half-maximizing
then maximizing after that.

I can reproduce this consistently (as well as #976627) by either using
Grid or clicking the maximize button on the window. The window is
invariably maximized in the left-most screen, while the window was
originally in the right-most screen.
