Yes, that seems to be what is happening. If I kill pulseaudio, and then
start totem, pulseaudio gets spawned again so the mixer starts working,
but the mute menu item remains greyed out.
I'm not sure why you think pulse isn't being restarted properly, as
everything else works.

Hmm, this sounds like Pulse is dying and not restarting properly. I did
notice something similar at a time in this cycle when there was alot of
work going on in dbusmenu (the library responsible for that button) but
not for a few weeks now.
