As I know it's rather a feature of pulseaudio then a bug. It let you use as
much volume controls of alsa as can to make you have more steps.
How you can change this behaviour:
1) http://ubuntuforums.org/showthread.php?t=1457462 to use just 1
control
or
2) you can change which controls to us
10.10 + Dell Latitude d620 = same issue. I have to use alsa-mixer to
adjust volume. And avoid touching of volume buttons.

Have same issue on Dell Inspuron 6400.
Do not remember having this on 10.04 and before, it started to happen after
upgrade to 10.10.
And addition to what Sridhar has said I can add that when I change volume in
volume control it reaches 100% in alsamixer well before I reach 'loudest'
position in
Updated to Ubuntu 10.10 and still have the issue, I had this problem
since 9.10
Using Dell Inspiron 640m.

I'll also note that I have not seen this problem on any of the other
computers I have used with Ubuntu. It seems specific to this Dell
Inspiron 9400.

1. open gnome-volume-control and gnome-alsamixer
2. in gnome-volume-control, drag the Output volume slider to 0 (but don't
mute)
3. start playing some audio in another application
4. slowly start dragging the Output volume slider up, and watch the Master
and PCM sliders in gnom
