This is not a PulseAudio bug. Some application is refusing to play
nicely; instead that app is using the audio device exclusively, and your
audio hardware is incapable of native multiopen.
If you're using the non-Free Adobe Flash plugin, you need to ensure that
you're using the correct Ubuntu conf

experimentation has shown that there is some sort of relationship with
firefox a recent update seemed to fix things somewhat
