Well, the easiest way to fix the app itself to it's original state was
just to use the package managed, and reinstall it. Also, does this
affect every application, so is it a buggy setting, or do just certain
applications respond badly to it?

package gnome-media.
The application you used to set the open was probably gconf-editor.
To restore the window menu you need to delete a file. Open the file
browser using "Places->Home Folder" and toggle the location
