Hi, sorry for not replying for so long. This bug was fixed through
Debian in version 1.13.3-4, which appeared in Jaunty as 1.14-1. Thus
it's been fixed for a long time so I'm marking this bug as Fix Released.

Yes, I can confirm this bug -- using lhs2tex without having stmaryrd.sty
doesn't work, and texlive-math-extra should therefore be a dependency.
